What are some mid range to high end sushi and noodle restaurants?

For sushi:
- Nemuro Hanamaru
It's a conveyor belt sushi restaurant but the price is rather mid-range. (I spent 4,000 yen to fill my stomach) They are originally from Hokkaido and the quality of fish is amazing.
It's in the shopping mall called KITTE near Tokyo Station and Tokyu Plaza in Ginza.

- Tsukiji Tamasushi
Tsukiji Tamasushi is located in multiple parts. Maybe the easiest one to stop by is the one in Shinjuku Takashimaya Department Store and Omotesando Hills.

For noodle shops, if you are looking for ramen shops, most of ramen shops are around 1,000 yen and below 2,000 yen. So there are not actually mid-range to high-end restaurants.
There are mid to high-end soba restaurants, though. Such as Muromachi Sunaba and Kanda Matsuya. They have not only amazing soba but also great side dishes, sake, and atmosphere.
